 | - The PreK-8 Homeschool Report Card Template is for students enrolling at Enlightium who were previously homeschooled for one or more years from grades PreK-8.
- If your student completed high school level courses during any of their middle school years, please also see “High School Homeschool Templates”.

High School Homeschool Template
 | - The High School Homeschool Transcript Template is for students enrolling at Enlightium who were previously homeschooled for one or more years from grades 9-12.
- One form must be provided and include information for each applicable high school year homeschooled.
- Additional requirements to validate credit:
- The High School Template must be signed by the parent.
- The High School Template must be signed by a local school official, or the following work samples* for each year of high school must be provided:
- English (1 essay and 2-4 other samples).
- History (1 essay/research paper and 2-4 other samples).
- Math (1 quiz/test and 2-4 lessons).
- Science (1 quiz/test and 2-4 other samples).
- Science Lab (1 sample of lab experiment or lab notes; 2-4 other samples).
- Electives: Art (3-5 art drawings and in-class work).
- Electives: Health (3-5 completed worksheets).
- Electives: Fitness (photos of sports and photos of students exercising).
- Electives: World Language (3-5 lessons).Electives: Other (worksheets, photos, and/or quizzes).
*All work samples need to include the student’s name, grade level, and course/subject information on each page. |
